After a severe acute nerve injury, neuronal fibers distal to the site will degenerate. As muscle fibers become denervated they will demonstrate spontaneous discharges (fibrillations and positive waves). These changes are first appreciated 7-10 days after a neuronal injury and can persist for a few months. If EMG is performed too early after an acute neuronal injury it can be falsely normal.
